Who Is Accountable When AI Makes a Decision?

Not who built the model. Not who trained it. Not who sold the software.

Who owns the decision?

AI is now influencing hiring, loan approvals, benefits eligibility, medical triage, fraud detection, and performance evaluations.

And in many organizations, no one can clearly name the human decision owner. That’s not a technical gap. That’s a leadership failure.

When accountability is unclear:

Every organization deploying AI should be able to answer three questions:

If you can’t answer those, you don’t have AI governance. You have automation without accountability — and that won’t hold up legally, operationally, or ethically.

AI does not remove responsibility. It concentrates it.

The organizations that understand this now will move faster, not slower. Trust accelerates adoption. The ones that don’t will be fixing damage in public. So when AI makes a decision in your organization — Who signs their name to it?
